A 100-bed skilled nursing facility in Sacaton with memory care, skilled nursing/hospice and long-term care units. Admission is by referral from a provider or hospital, and residents must be enrolled in the Arizona Long Term Care System. Admissions office is open 7 days, 8-5; email tchadmissions@grhc.org.
